It is represented by a gray cross with a red skull. This medal is only awarded if the entire opposing team is exterminated.

This medal is extremely hard to get on purpose as you rarely find all of the same team together, and it is hard to stay alive killing at least four enemies within four seconds, each within those four seconds.
 
This is most commonly received on Team SWAT, as you only have to get headshots, there is also a possibility of obtaining this in [[Grifball]]. Another easy way to get this is having the [[Rocket Launcher]] (or any other explosive weapon) and hope the other team is near each other in Ranked Team Slayer game, due to their being four opponents, not five like in Social Slayer, but sometimes by luck using the [[Spartan Laser]] is also another way.
In the first three appearances, it is a Legendary medal awarded for killing the last alive player on the opposing team with at least an [[Overkill Medal|Overkill]] that results in the entire team being dead at once. In the first two games, it is represented by a gray cross with a red skull, and in ''Halo 4'' it represented by a white skull on a red hexagon on top of a 16-pointed grey star.

In ''Halo Infinite'', it is a Mythic medal which is awarded for solely wiping the enemy team in a round of [[Elimination]].

This medal can be considered very difficult to earn, requiring a player to not only encounter four enemies together but also successfully and quickly kill the team. The medal is likely more common in game modes such as [[SWAT]] as time-to-kill can be instantaneous.
 
It can also be obtained by killing the person who is in a team consisting only of himself, four times within four seconds of each kill.
