A good way to counter this is to sneak around the guy, go up the ladder and smack the poor sucker in the back.
Another name for this tactic is "'nade spamming"

By first hand experience in Halo 2 at least, when the grenade was shot, it didn't explode, it was only bounced by the bullet so that it would change course.
