| The Gravemind can't be Keyes, because Keyes was on the Truth and Reconciliation, which was, according to the books, destroyed. Also, right after that, according to the books AND the game, Halo was destroyed, further destroying any remains of Halo. Finally, Gravemind is on Installation 05, which is probably at least 25,000 light years away from the remains of Keyes as a brain form, and the events of Halo 2 are very soon after Halo.
