The Pistol Perfect medal is awarded in the Halo: Spartan Assault and Halo: Spartan Strike campaigns for killing six enemies in a row, with a pistol, without being hit.
Only the Magnum, Plasma Pistol and Needler count toward this medal; the Dual SMGs do not.
Trivia
The name of the medal is likely a reference to the term "picture perfect".
