:If you're playing on legendary when you get the skull, the effect should activate immediately. When you pick it up, the screen should flash white, most of your HUD will disappear, and the skull's name (if any) will appear in the upper left corner of the screen. You should also hear a type of 'fly-by' sound. If you don't get this, you either found the skull but weren't playing on legendary, or somehow glitched it to not work or something.
